What is Paroxysmal Hemicrania?

Paroxysmal hemicrania is a rare primary headache type in the trigeminal autonomic cephalalgias group, characterized by unilateral, severe, and short-lasting attacks. Attacks may recur from a few times to 40 times per day.

Each attack usually lasts 2-30 minutes and is associated with severe pain localized around the eye along with autonomic findings on the same side such as tearing, eye redness, nasal discharge, or congestion.

Diagnosis is largely made by definite response to indomethacin therapy. In cases that do not respond to treatment, secondary causes should be investigated.

Symptoms

Unilateral severe pain centered around the eye
Short attacks lasting 2-30 minutes
5 or more attacks per day
Tearing and redness on the same side
Nasal congestion or discharge
Eyelid swelling or drooping (ptosis)
Restlessness and tendency to move

Risk Factors

Female sex (more common)
Age 20-40
Family history of headache
Stress and irregular sleep
Alcohol consumption
Onset after trauma
Rarely underlying structural lesions

  • If short and recurrent severe headaches occur during the day
  • If headache seriously affects work and sleep
  • Neurology evaluation is essential before indomethacin
  • If autonomic findings appear on the same side in each attack
  • If response cannot be obtained from current treatments
