What is Osteoporosis Exercise Rehabilitation?

Osteoporosis is a systemic skeletal disease characterised by decreased bone mineral density and disruption of microarchitecture, increasing fracture risk.

Exercise rehabilitation is one of the most important non-pharmacological interventions in osteoporosis. Mechanical loading triggers osteoblast activity through Wolff's law, increasing bone density.

Effective programmes include progressive resistance training (high intensity), impact loading exercises (jumping, hopping), balance training and posture exercises. The LIFTMOR (Lifting Intervention for Training Muscle and Osteoporosis Rehabilitation) protocol uses 80–85% 1RM resistance training.

Symptoms

Asymptomatic in early stage (silent disease)
Loss of height and stooped posture (kyphosis)
Acute or chronic back pain (vertebral fracture)
Wrist, hip, vertebra fragility fracture
Decreased mobility and fear of falling
Quality of life impairment
Decreased grip strength

Risk Factors

Postmenopausal women (oestrogen deficiency)
Advanced age (>65 years)
Family history of osteoporosis
Low body weight (BMI <19)
Smoking and alcohol use
Long-term corticosteroid use
Calcium and vitamin D deficiency

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Bone density (DEXA) screening should start at age 65 for postmenopausal women
  • Apply to the emergency department immediately for sudden back pain with possible vertebral fracture
  • Hip pain after a fall requires X-ray investigation
  • Frequent falls require comprehensive geriatric evaluation
