What is Osteopathy — Somatic Dysfunction Treatment?

Osteopathy was founded by A.T. Still in the late 19th century and is based on the principle of structure-function unity of the body.

Somatic dysfunction is impairment of the function of the somatic system components (skeleton, joint, myofascial, vascular, neural).

Manual techniques (HVLA, muscle energy, myofascial release, counterstrain) are applied; in the USA DO equivalent to MD.

Symptoms

Acute or chronic low back pain
Cervical and shoulder pain
Tension type headache
Joint motion limitation
Muscle stiffness and trigger point pain
Postural disturbance
Pelvis-sacrum dysfunction

Risk Factors

Caution should be exercised in the presence of severe osteoporosis (HVLA contraindicated)
Vertebrobasilar insufficiency: caution in cervical manipulation
Cancer with bone metastasis is contraindicated
Acute fracture and inflammatory arthritis exclude direct technique
Vascular intervention is risky during anticoagulant use
Vertebral artery dissection: cervical HVLA risk
Spinal infection (osteomyelitis) is absolute contraindication

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Saddle anaesthesia, urinary retention or weakness must be evaluated immediately for spinal cord pathology
  • If pain persists for >6 weeks, consultation with neurology and orthopaedics is recommended
  • Cancer-related back pain mandates oncology evaluation
  • Definite imaging is essential for trauma history
