What is Opsoclonus-Myoclonus Syndrome?

Opsoclonus-myoclonus syndrome (OMS) is a rare neurological disorder defined by rapid, irregular, multidirectional eye movements (opsoclonus) together with brief involuntary muscle contractions (myoclonus).

In children it is frequently associated with neuroblastoma, while in adults it may be a paraneoplastic syndrome of lung, breast, or ovarian cancer.

Autoimmune mechanisms are central to its pathogenesis; antineuronal antibodies damage cerebellar and brainstem structures.

Symptoms

Rapid, chaotic, multidirectional eye movements (opsoclonus)
Sudden muscle spasms in the trunk and limbs (myoclonus)
Ataxia with impaired balance and coordination
Irritability and disturbed sleep, especially in children

Risk Factors

Neuroblastoma — the leading cause in children
Lung or breast cancer in adults
Postviral autoimmune triggering

  • Sudden motor regression and irritability in a child
