What is Ophthalmoplegic Migraine (Recurrent Painful Ophthalmoplegic Neuropathy)?

Ophthalmoplegic migraine is a rare entity characterized by unilateral severe headache accompanied by transient oculomotor (usually third cranial nerve) palsy. In current classifications it is considered 'recurrent painful ophthalmoplegic neuropathy'.

Clinically, eyelid drooping, restricted eye movements, and double vision occur with the attacks. Symptoms are seen mostly in children and young adults.

Serious causes such as aneurysms, tumors, and infections must be excluded in the diagnosis. The first attack requires emergency imaging and neurological evaluation.

Symptoms

Unilateral severe headache
Eyelid drooping (ptosis)
Restricted eye movements
Double vision
Pupil dilation
Nausea, vomiting
Attacks lasting several days and recurring

Risk Factors

Childhood and young adulthood
History of migraine-like headache
Family history of migraine
Hormonal changes
Stress and sleep irregularity
Recent minor infections
Individual triggers
