What is Neuromuscular Electrical Stimulation (NMES)?

Neuromuscular electrical stimulation (NMES) is the method of producing physiological muscle contraction by stimulating peripheral nerves and motor end plates with surface or implanted electrodes via low-frequency electric current (1 to 100 Hz).

Mechanism: Excitation of motor neurons elicits muscle contraction; recruits primarily fast (Type II) muscle fibres.

Indications: post-stroke hemiplegia, post-spinal cord injury muscle atrophy, post-knee surgery quadriceps strengthening, dysphagia, urinary incontinence (pelvic floor).

Symptoms

Muscle atrophy and weakness
Decreased motor function (post-stroke hemiplegia)
Postsurgical immobilisation period (knee, shoulder)
Disuse atrophy (intensive care, fracture)
Spasticity and motor control disorder
Dysphagia and pelvic floor weakness

Risk Factors

Implanted cardiac pacemaker and defibrillator
Pregnancy (over the abdomen)
Active malignancy (in the application area)
Skin lesion, infection and sensory deficit
Severe deep vein thrombosis
Uncontrolled epilepsy and severe heart disease

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Physiatry evaluation in muscle weakness and motor function loss
  • Sensory testing and skin examination before NMES
  • Cardiologist consultation in those with cardiac pacemaker
  • Personalised programme by physiotherapist supervision

Treatment Methods

01
Personalised parameter setting (frequency, pulse width, intensity)
02
20 to 30 minute session, 3 to 5 times a week, 8 to 12 weeks
03
Functional electrical stimulation (FES, integrated with movement)
04
Combination with active and passive exercise
05
Pulse width 200 to 400 microseconds (typical)
06
Frequency 30 to 50 Hz (for muscle strength)
