What is Neck Pain — Postural Rehabilitation?

Chronic neck pain (>3 months) is one of the most common musculoskeletal problems and increases significantly with screen-related work. The most common cause is forward-head posture (FHP) and upper crossed syndrome (UCS).

FHP increases the load on the cervical spine; the head shifts 10° forward in normal posture, and the load on the spine increases tenfold. Ergonomic factors and weakness of the deep cervical flexors are the key issues.

Treatment principles include posture correction, deep cervical flexor strengthening (chin tuck), trapezius and pectoral stretching, scapular stabilisation and ergonomic regulation. The McKenzie method also provides cervical extension exercises.

Symptoms

Persistent neck pain and stiffness
Headache (cervicogenic)
Trapezius and shoulder muscle pain
Numbness and tingling sensation in the arm
Reduced cervical range of motion
Forward-head posture and round shoulders
Stress and tension increase pain

Risk Factors

Long-term computer use
Smartphone use ('text neck')
Sedentary lifestyle
Poor sleep posture
Stress and emotional tension
Previous neck trauma (whiplash)
Female sex and middle age

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Apply to the emergency department for severe neck pain with weakness, numbness or urinary incontinence (myelopathy)
  • Investigation needed for neck pain with fever, weight loss and night sweats
  • Cancer history with new-onset neck pain requires screening
  • Specialist referral needed for pain that does not respond to 6-week conservative treatment
