What is Myasthenic Crisis Management?

Myasthenic crisis is an acute exacerbation of myasthenia gravis severe enough to require respiratory support due to weakness of bulbar or respiratory muscles.

It is most often triggered by infection, surgery, certain medications or non-compliance with treatment.

Mortality has decreased significantly with modern intensive care management.

Symptoms

Worsening dyspnea and tachypnea
Inability to clear secretions and swallowing difficulty
Weakness of neck and respiratory muscles
Decrease in vital capacity below 1 liter or 15 ml/kg
Generalized fatigue and ptosis worsening

Risk Factors

Worsening myasthenia gravis or recent diagnosis
Active infection, especially respiratory tract infection
Recent surgery or trauma
Magnesium, fluoroquinolones, beta blockers and aminoglycosides
Discontinuation of immunosuppressive therapy

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Severe respiratory difficulty or aspiration findings — emergency
  • Vital capacity falling below 20 ml/kg
  • Bulbar weakness with risk of choking
  • Severe dysphagia preventing oral intake

Treatment Methods

Hospitalization in intensive care and frequent monitoring of vital capacity

Endotracheal intubation and mechanical ventilation when necessary

Plasmapheresis or IVIG infusion for rapid response

High-dose corticosteroids and immunosuppressive therapy

Treatment of triggering factor and discontinuation of provoking medications
