What is MuSK Antibody-Positive Myasthenia Gravis?

MuSK (muscle-specific tyrosine kinase) antibody-positive myasthenia gravis is a subtype that accounts for a portion of AChR-negative patients and shows clinical features partially distinct from classic myasthenia gravis.

In these patients, the bulbar, neck, shoulder, and respiratory muscles are predominantly involved, while ocular findings may be less prominent.

MuSK antibodies belong to the IgG4 subclass and disrupt the clustering of acetylcholine receptors at the neuromuscular junction.

Symptoms

Marked dysphagia and dysarthria
Facial muscle weakness and atrophy
Early neck-flexor involvement
Frequent episodes of respiratory failure
Relatively mild ocular findings
Fatigue worsened by exertion
Tongue atrophy in some patients

Risk Factors

Female sex (clear majority)
Background of autoimmune disease
HLA-DR14, DR16 tissue types
