What is Muscle Strength Evaluation by Manual Muscle Testing (MMT)?

Manual muscle testing is an examination technique standardized by Daniels and Worthingham, in which the strength of a muscle or muscle group is evaluated manually by a physiatrist.

Evaluation is graded between 0 (no contraction) and 5 (normal strength). Grade 3 (full motion against gravity) is a key point; below this it is evaluated in a gravity-eliminated position, and above it against gravity and against resistance.

MMT is used in the diagnosis, treatment planning and follow-up of rehabilitation effectiveness in neurological and musculoskeletal diseases. It can be supported by instrumental measurements such as a dynamometer.

Symptoms

Muscle weakness
Hemiplegia and hemiparesis
Neurological deficit
Peripheral nerve injury
Myopathy
Muscle atrophy
Gait disturbance

Risk Factors

Stroke and cerebrovascular event
Spinal cord injury
Peripheral neuropathies
Muscular dystrophy
Myasthenia gravis
Long-term immobilization
Sarcopenia of aging

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When a decrease in muscle strength is felt
  • In neurological disease follow-up
  • At the start of a rehabilitation program
  • In periodic rehabilitation follow-ups
  • In preoperative evaluation

Treatment Methods

01
Grading with the standard MMT scale
02
Isometric muscle testing
03
Dynamometer measurement (hand grip strength)
04
Measurement with isokinetic device
05
Functional muscle tests
06
Use in treatment planning
