What is Multiparametric MRI of the Prostate?

Multiparametric prostate MRI (mpMRI) combines T2-weighted anatomical imaging with diffusion-weighted imaging (DWI) and dynamic contrast-enhanced (DCE) sequences on 3 Tesla scanners with surface or endorectal coils. It is the most accurate non-invasive method for detecting clinically significant prostate cancer.

PI-RADS v2.1 standardizes lesion scoring from 1 (very low) to 5 (very high), guiding the need for targeted biopsy. mpMRI prior to biopsy reduces unnecessary biopsies, increases detection of significant disease and is endorsed by EAU and AUA guidelines.

Symptoms

Elevated PSA without prior biopsy (PSA above 4 ng/mL or age-adjusted threshold)
Continuous PSA rise after negative TRUS biopsy
Active surveillance protocol monitoring
Suspicious DRE finding
Biochemical recurrence after prostatectomy or radiotherapy
Pre-operative staging in confirmed cancer

Risk Factors

Age over 50 (over 45 if positive family history)
African ancestry
BRCA1/2 or HOXB13 germline mutation
Family history of prostate cancer
High dietary fat and processed meat intake
Obesity and metabolic syndrome

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Elevated PSA without prior imaging or biopsy
  • Suspected cancer despite normal initial workup
  • Active surveillance reassessment at 1-2 year intervals
  • Pre-treatment staging in newly diagnosed cancer
  • Suspected local recurrence on rising PSA after definitive therapy
