What is Multi-Layer Compression Bandaging in Lymphedema?

Multi-Layer Lymphedema Bandaging (MLLB) is a four-layer bandaging system applied by a certified lymphedema therapist. The layers are: 1) skin-protective cotton tube, 2) finger bandage, 3) synthetic cotton padding (cone shape), 4) short-stretch compression bandage.

Short-stretch bandages provide low resting pressure and high working pressure. This optimizes lymphatic drainage together with muscle pump activity.

The bandage is left on for 24 hours, changed daily, and applied continuously during the intensive phase (2–4 weeks). Correct technique (uniform pressure, conical shape, correct tension) is the key to success. Incorrect application can cause constriction and skin damage.

Symptoms

Active phase lymphedema
Marked limb swelling
Skin firmness
Need for the intensive phase of CDT
Insufficiency of compression garments
Pitting edema

Risk Factors

Incorrect application and constriction
Arterial insufficiency
DVT
Skin sensitivity
Patient intolerance
Infection

  • When compression garments become insufficient
  • In preparation before surgery
