What is MRCP (MR Cholangiopancreatography)?

MRCP (magnetic resonance cholangiopancreatography) provides imaging with heavy T2-weighted sequences that detect static fluid in bile and pancreatic ducts as signal. Contrast material is mostly not necessary.

It is used in cholelithiasis, choledocholithiasis, primary sclerosing cholangitis, pancreatic ductal anomalies, papilla tumors and post-surgical biliary evaluation. It is not therapeutic compared to ERCP but has high diagnostic safety.

Patients usually fast for 4-6 hours and the procedure takes about 20-30 minutes. Careful planning is required in cases of magnetically compatible implants and claustrophobia.

Symptoms

Cholelithiasis and choledocholithiasis
Differential diagnosis of jaundice
Primary sclerosing cholangitis
Chronic pancreatitis and ductal anomalies
Suspicion of bile duct cancer
Post-surgical biliary evaluation

Risk Factors

MR contraindication (incompatible metallic implant)
Severe claustrophobia
Severe renal failure (if contrast will be given)
High motion and breath-holding difficulty
Pregnancy (first trimester relative)
Very high obesity

When to See a Doctor?

  • Planned with gastroenterology or surgery specialist referral
  • Used as a diagnostic confirmation tool before ERCP
  • Clinical evaluation is important in acute cholangitis and colicky pain
  • Results should be interpreted together with clinical laboratory
