What is MR Spectroscopy (Detailed)?

MR spectroscopy (MRS) is a noninvasive MRI-based technique that detects nuclear-magnetic-resonance signals from key tissue metabolites such as N-acetylaspartate (NAA), choline, creatine, lactate, lipids, citrate and myo-inositol.

Single-voxel and chemical-shift imaging acquisitions are tailored to the clinical question. Spectra are interpreted as ratios (Cho/NAA, Cho/Cr) or absolute concentrations.

Applications include brain tumor grading and follow-up, suspected mitochondrial or metabolic disease, hepatic encephalopathy and prostate cancer characterization with multiparametric MRI.

Newly diagnosed brain tumor — characterization and grading
Distinguishing tumor recurrence from radiation necrosis
Suspected mitochondrial or metabolic disorder
Atypical demyelinating disease
Hepatic encephalopathy assessment
Prostate cancer staging in selected cases

Risk Factors

Standard MRI contraindications (devices, severe claustrophobia)
Patient cooperation (motion-sensitive)
Voxel placement near bone or air can degrade spectra
Iron deposition disorders impair signal
Metallic implants in the field of view

When to See a Doctor?

  • Indeterminate intracranial mass on conventional MRI
  • Post-treatment lesion progression
  • Suspected leukodystrophy or organic acidemia
  • Pediatric developmental regression with imaging needs
