MR Elastography for Liver Fibrosis Assessment

What is MR Elastography for Liver Fibrosis Assessment?

MR elastography (MRE) is a non-invasive quantitative MRI technique that measures liver tissue stiffness by applying low-frequency mechanical shear waves (40-100 Hz) through a passive driver placed on the right upper quadrant.

The induced shear wave propagation is captured using phase-contrast MRI with motion-encoding gradients, generating elastograms color-coded for tissue stiffness expressed in kilopascals (kPa). Modern 3T systems with 3D MRE provide superior accuracy and reduced acquisition time.

Diagnostic thresholds: F0-F1 (no/mild fibrosis) <2.93 kPa, F2 (significant fibrosis) 2.93-3.55 kPa, F3 (advanced fibrosis) 3.55-4.62 kPa, F4 (cirrhosis) >4.62 kPa. AUC ranges 0.94-0.98 for F2-F4, surpassing FibroScan transient elastography (AUC 0.85-0.93) and serum biomarkers.

Hemochromatosis or Wilson disease
Suspected cirrhosis with portal hypertension
Post-transplant liver graft assessment
Hepatocellular carcinoma surveillance candidate identification

Risk Factors

Chronic viral hepatitis (HBV, HCV)
Metabolic syndrome and obesity (BMI >30)
Type 2 diabetes mellitus
Significant alcohol consumption (>30g/day men, >20g/day women)
Family history of liver disease or hereditary hemochromatosis
Long-term hepatotoxic medication use
Autoimmune liver disease
Persistently elevated liver enzymes

When to See a Doctor?
