What is MR Arthrography Shoulder Labrum Evaluation?

MR arthrography is magnetic resonance imaging performed after dilute gadolinium injection into the joint space under fluoroscopy or ultrasound guidance. The distension of intra-articular structures provides superior diagnostic performance compared to conventional MR.

It is the gold standard method for diagnosis of shoulder labral tears (SLAP, Bankart lesions), partial rotator cuff tears, capsular pathologies, and detection of loose bodies.

Two techniques are used: direct MR arthrography (intra-articular contrast) and indirect MR arthrography (intravenous contrast followed by exercise). Direct method has higher accuracy.

Symptoms

Recurrent shoulder dislocation and instability
Shoulder pain and locking sensation
Indeterminate labral findings on conventional MR
Athletes' shoulder injuries (suspicion of SLAP lesion)
Partial rotator cuff tear evaluation

Risk Factors

Risk of intra-articular infection (very low)
Gadolinium allergy
Local pain and discomfort
Coagulopathy
