What is Medication Overuse Headache?

Medication overuse headache (MOH) is a secondary headache type that develops in people with primary headache disorders due to excessive and regular use of medications taken for treatment. It should be considered when there are 15 or more headache days per month for over three months.

Risk arises when simple analgesics are used more than 15 days a month, and triptans and opioids more than 10 days a month. Over time, the headache becomes chronic and intensifies when the medication is reduced.

The mainstay of treatment is gradual withdrawal of the offending medication, initiation of alternative preventive therapy, and patient education. The process can be difficult for patients, but the prognosis is generally good.

Symptoms

Headache almost every day
Headache present upon waking in the morning
Pain that intensifies as the medication wears off
Increasing medication need
Difficulty concentrating and fatigue
Insomnia and anxiety
Depressed mood

Risk Factors

History of chronic migraine or tension-type headache
Use of triptans, opioids, combined analgesics
Use of pain relievers more than 10-15 days per month
Chronic pain conditions
Anxiety and depression
Stressful life events
Insomnia and physical inactivity
