What is Maxillary Sinus Perforation During Dental Implant Surgery?

Maxillary sinus perforation occurs when the floor of the maxillary sinus or the Schneiderian membrane is breached during posterior maxillary dental implant placement, often due to insufficient bone height (less than 5-7 mm), aggressive osteotomy, or unrecognized sinus pneumatization.

Risk increases with edentulous posterior maxilla, prior tooth extraction with bone resorption, chronic sinus disease, and atypical sinus anatomy. Pre-operative cone-beam CT (CBCT) is essential to plan implant length and need for sinus lift.

Small perforations (under 3 mm) often heal with conservative measures; larger perforations require sinus lift with membrane repair (collagen membrane, sutures), antibiotics, sinus precautions, and possibly staged implant placement after healing.

Symptoms

Bleeding from nose after implant surgery (epistaxis)
Air or fluid passage between mouth and nose
Implant mobility or loss
Sinusitis symptoms (facial pain, nasal congestion, postnasal drip)
Halitosis or unpleasant taste
Visible perforation on CBCT or panoramic imaging
Foreign body sensation (displaced implant in sinus)

Risk Factors

Insufficient bone height in posterior maxilla (<5-7 mm)
Long-term edentulism with bone resorption
Maxillary sinus pneumatization
Chronic sinus disease or polyps
Surgical inexperience or aggressive osteotomy
Inadequate pre-operative imaging
Smoking and poor wound healing
