What is Manual Lymphatic Drainage?

Manual lymphatic drainage (MLD) is a physical therapy technique consisting of light-pressure, directional movements applied to the skin to facilitate lymphatic flow. It is one of the basic steps of complex decongestive therapy (CDT).

There is evidence for its use in arm lymphedema after breast cancer surgery, lower extremity lymphedema and primary lymphedema. It is applied together with bandaging, exercise and skin care.

In Turkiye it should be performed by a certified physiotherapist or a CAM-authorized (GETAT) physician. It may be contraindicated in heart failure and active infection.

Symptoms

Arm lymphedema after breast cancer surgery
Lower extremity lymphedema
Primary lymphedema
Edema after trauma
Chronic venous insufficiency (supportive)
Postoperative edema management

Risk Factors

Congestive heart failure
Active cellulitis and erysipelas
Deep vein thrombosis
Untreated active cancer in the local area
Acute kidney failure
Widespread skin infection

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Consult a physician when signs of lymphedema are noticed
  • Sudden swelling and pain in a limb should be evaluated for thromboembolism
  • Patients with skin infection should not undergo MLD without treatment
  • The treatment plan must be designed carefully in heart and kidney failure
