Lymphoedema and Pneumatic Compression Therapy

What is Lymphoedema and Pneumatic Compression Therapy?

Lymphoedema is a chronic accumulation of protein-rich interstitial fluid in tissue caused by lymphatic system insufficiency.

Causes: post-mastectomy, post-cancer radiotherapy, surgical lymph-node dissection, congenital lymphatic system abnormality and filariasis.

Pneumatic compression devices have multi-chamber sleeves that work in sequential pattern (proximal to distal); they support manual lymphatic drainage and become a part of complete decongestive therapy.

Symptoms

Indications: post-mastectomy upper limb lymphoedema
Indications: post-pelvic surgery lower limb lymphoedema
Indications: post-deep vein thrombosis chronic venous insufficiency
Indications: heart-failure-related extremity oedema
Caution: active deep-vein thrombosis (DVT)
Caution: severe heart failure (NYHA III-IV)
Caution: skin infection (cellulitis)

Risk Factors

Post-mastectomy axillary lymph-node dissection
Pelvic radiotherapy and pelvic surgery
Obesity (BMI >30) and overweight
Cellulitis history (recurrent infection)
Chronic venous insufficiency and varicose veins
Long-term immobilisation
Heart failure and renal failure

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Pneumatic compression must be applied under physiotherapist or lymphoedema specialist supervision
  • DVT, infection and severe heart failure must be excluded
  • Daily session: 30-60 min, 5 days a week and weekly review
  • Combined therapy: complete decongestive therapy (CDT)
