What is Lower Extremity Prosthesis Training?

Lower extremity prosthesis training is a rehabilitation program tailored to the type of prosthesis (transtibial or transfemoral) after amputation and to the K-level (K0–K4 activity level). Prosthesis fit, gait quality and functional independence are the main goals.

K-classification: K0 (cannot use a prosthesis), K1 (can walk at home), K2 (limited community ambulation), K3 (independent community ambulation, variable cadence), K4 (high activity, athlete). Prosthesis components are selected according to activity level.

Training progresses from simple to complex: weight shifting in parallel bars, single steps, walking outside the parallel bars, stairs, ramps, uneven ground and finally running and sport activities.

Symptoms

Difficulty walking with the prosthesis
Gait deviations
Imbalance and risk of falling
Stump pain (prosthesis fit problem)
Phantom pain flare-ups
Increased energy expenditure

Risk Factors

Transfemoral amputation (TF > TT difficulty)
Older age
Bilateral amputation
Vascular amputation (low conditioning)
Difficulty in psychological adaptation
Inappropriate prosthesis selection

  • When the risk of falling increases
  • When transitioning to advanced activities
