Low Back Pain — McKenzie Method

What is Low Back Pain — McKenzie Method?

The McKenzie Method (Mechanical Diagnosis and Therapy, MDT) was developed by physiotherapist Robin McKenzie in 1981 as a system for chronic low back, neck and limb pain.

The principle of directional preference is investigated by repeated movement test: the direction (extension, flexion, lateral) where the patient's pain centralises (away from the limb) is identified.

Most patients have extension preference; standing, prone position and lumbar lordosis exercises are used. Effective in derangement syndrome (disc origin) according to the McKenzie classification.

Symptoms

Chronic low back pain (>3 months)
Sciatica (pain radiating to the leg)
Pain increases on prolonged sitting
Pain that is worse in the morning, eases with movement
Numbness and tingling sensation in the leg
Difficulty bending forward
Pain centralisation (with directional movement test)

Risk Factors

Sedentary lifestyle
Sitting work and bad posture
Heavy lifting and repetitive movement
Obesity (BMI >30)
Smoking (impairs disc nutrition)
Family history (genetic predisposition)
Previous low back injury

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Apply to the emergency department immediately for sudden severe low back pain with leg weakness or urinary incontinence (cauda equina)
  • Investigation needed for low back pain with fever, weight loss and night sweats
  • Investigation needed for pain that does not respond to 6-week conservative treatment
  • Cancer history with new-onset low back pain requires screening
