What is Low Back Pain Exercise Program?

Low back pain (LBP) affects 70-85% of adults at some point in life; chronic LBP develops in 10-15%. Most cases (85-90%) are non-specific - no specific anatomical cause; mechanical, postural, and biopsychosocial factors are responsible. Exercise therapy is the first-line treatment for chronic LBP; pharmacotherapy is supplementary. Stuart McGill, Big 3 (curl-up, side bridge, bird-dog) is widely used.

Exercise types: 1) Motor control - lumbopelvic neuromuscular control, transversus abdominis activation; 2) McKenzie - directional preference (extension or flexion-based) determination, centralization phenomenon; 3) Core stabilization - dynamic spine stabilization (multifidus, transversus abdominis, diaphragm, pelvic floor); 4) Aerobic - low-impact (walking, swimming, biking) cardiopulmonary improvement; 5) Stretching and flexibility - hamstring, hip flexor stretching.

Progressive protocol: Week 1-2 - pain modulation, gentle ROM, neutral spine education; Week 3-4 - motor control basic, isometric core; Week 5-8 - dynamic core, McKenzie progression; Week 9-12 - functional movement (squat, deadlift modification), aerobic conditioning. Frequency: 3-5 times/week, 30-60 minutes. Pain principle: 'no harm pain' rule, VAS <4/10. Recurrence prevention: lifelong exercise habit, ergonomics, weight control, smoking cessation.
