What is Liver Elastography for Fibrosis?

Liver fibrosis stage is the strongest prognostic marker in chronic liver disease and treatment decisions; while liver biopsy is the historic gold standard, sampling error and complication rates have led to the development of noninvasive alternatives. Transient elastography (FibroScan) measures velocity by sending a 50 Hz mechanical pulse into the liver tissue (m/s, kPa); cutoffs depend on the etiology.

Point shear wave elastography (pSWE, ARFI) and 2D shear wave elastography (2D-SWE) are integrated into ultrasound devices; they have higher technical success and provide larger sampling areas. MR elastography (MRE) is the most accurate noninvasive method (AUC 0.92-0.98) and works in obese patients and ascites.

FibroScan failure rates exceed 5-10% in the obese (BMI > 30); the XL probe addresses this. Vibration-controlled transient elastography (VCTE) is rapid and reproducible. Cutoffs vary by etiology: in chronic HCV, F2 ≥ 7.1, F3 ≥ 9.5, F4 ≥ 12.5 kPa.

  • Persistent transaminase elevation lasting more than 6 months
  • Chronic hepatitis B/C diagnosis
  • Hepatosteatosis on imaging
  • Risk factors (obesity, diabetes)
  • Treatment response follow-up (post-DAA)
