What is Lambert-Eaton Myasthenic Syndrome?

Lambert-Eaton myasthenic syndrome (LEMS) is an autoimmune disease of the neuromuscular junction characterized by antibodies against voltage-gated calcium channels at nerve terminals, which reduces acetylcholine release.

About half of cases are associated with malignancy such as small cell lung cancer (paraneoplastic LEMS). The other half occurs as an isolated autoimmune disorder.

Clinically, proximal muscle weakness, brief facilitation with activity, autonomic findings such as dry mouth and constipation, and loss of reflexes are typical. Treatment includes searching for underlying cancer and improving neuromuscular transmission.

Symptoms

Proximal muscle weakness (especially in the legs)
Difficulty climbing stairs and rising from a chair
Brief facilitation after exertion
Dry mouth and metallic taste
Constipation and sexual dysfunction
Loss of deep tendon reflexes
Ptosis and diplopia (less frequent)

Risk Factors

Small cell lung cancer
Other autoimmune diseases
Smoking
Advanced age
Male sex (in paraneoplastic form)
Family history of autoimmune disease
HLA susceptibility

When to See a Doctor?

  • Concurrent proximal weakness and autonomic symptoms
  • New-onset weakness in a patient with smoking history
  • Failure to respond to myasthenia gravis treatment
  • Accompanying unexplained weight loss and fatigue
  • New neurological symptoms in cancer patients under regular follow-up
