What is Intraoral Soft Tissue Lesions?

Intraoral soft tissue lesions arise on the buccal mucosa, lips, gingiva, palate, tongue and floor of mouth. Reactive lesions (irritation fibroma, peripheral giant cell granuloma, pyogenic granuloma) are the most common and follow chronic mechanical or chemical irritation.

Premalignant lesions include leukoplakia (white patches that cannot be wiped off) and erythroplakia (red patches), which carry a substantial risk of progression to squamous cell carcinoma. Lichen planus, particularly the erosive form, has malignant potential and requires monitoring.

Other lesions include traumatic ulcers, mucocele, pyogenic granuloma, papilloma, hemangioma and pemphigus. Persistent ulcers beyond 2 weeks, indurated lesions, and lesions in high-risk patients (smoking, alcohol) warrant biopsy.

Symptoms

White or red patches in the mouth
Persistent oral ulcer beyond 2 weeks
Painless lump or swelling
Bleeding or non-healing lesion
Hardness or induration of the lesion
Numbness in the mouth or lips
Difficulty chewing, swallowing or speaking

Risk Factors

Tobacco use (smoking, chewing, snuff)
Heavy alcohol consumption
Betel nut chewing
Human papillomavirus infection
Chronic mechanical irritation (sharp tooth, ill-fitting denture)
Lichen planus or autoimmune disease
Immunosuppression or HIV infection

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Persistent oral ulcer or lesion beyond 2 weeks
  • White or red patch that does not wipe off
  • Indurated or hard mass
  • Non-healing lesion or persistent bleeding
  • Numbness or persistent pain
  • Lesion in tobacco or alcohol user
