What is Impacted Third Molar (Wisdom Tooth) Surgery?

Impacted third molar surgery involves the surgical removal of wisdom teeth that fail to erupt fully due to insufficient space, malposition or pathology. Pell-Gregory and Winter classifications guide surgical complexity based on depth and angulation (mesioangular, distoangular, vertical, horizontal).

Modern protocols use 3D CBCT for inferior alveolar nerve relationship assessment, piezoelectric surgery for selected cases, coronectomy for high-risk nerve proximity and platelet-rich fibrin to enhance healing and reduce dry socket risk.

Symptoms

Recurrent pericoronitis with operculum inflammation
Caries on the second molar distal surface
Periodontal pocket on the distal of second molar
Cyst or tumor associated with the impacted tooth
Pain or pressure radiating to the jaw and ear
Trismus and difficulty in mouth opening
Halitosis from food impaction

Risk Factors

Mandibular ramus retrognathia
Distal angulation or horizontal impaction
Smoking (delayed healing, dry socket)
Poor oral hygiene with chronic pericoronitis
Diabetes mellitus and immunosuppression
Long-term bisphosphonate therapy
Coagulopathy or anticoagulant use

  • Recurrent gum infection or pain around the wisdom tooth
  • Detected impaction on routine dental radiograph
  • Orthodontic planning requiring extraction
  • Caries or periodontal compromise of the adjacent second molar
  • Pre-emptive removal in high-risk young adults
  • Pathologic findings (cyst, tumor) on imaging
