What is Image-Guided Radiologic Drainage?

Radiologic drainage involves inserting a thin catheter through the skin under imaging guidance to evacuate fluid collections (abscess, hematoma, pleural effusion, ascites). It is a low-complication alternative to open surgery.

Suitable locations include pleural effusion, pericardial effusion, hepatic abscess, abdominal collections, and urinary tract obstruction.

The procedure is usually done under local anesthesia; patient tolerance is high and hospital stay is short.

Symptoms

Fever and suspected abscess
Pain and fever with a detected collection
Need to evacuate a pleural effusion
Need for percutaneous nephrostomy
Abdominal collections (post-operative or infected)
Need for biliary drainage

Risk Factors

Coagulopathy
Diffuse infection
Uncooperative patient
Anatomic difficulty (obesity, interposed organs)
Anticoagulant use
Immunosuppression

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Severe pain, fever and signs of abscess require radiology evaluation
  • Drainage is considered for pleural effusion accompanied by dyspnea
  • Severe pain, bleeding or fever after catheter placement is an emergency
  • Rapid drainage is needed when biliary obstruction is accompanied by jaundice
