What is Hypnic Headache?

Hypnic headache (alarm clock headache) is a rare primary headache that wakes the person from sleep and occurs only during sleep. It is usually seen in individuals over the age of 50.

Attacks usually start at the same time of night, can last from 15 minutes to 4 hours, and are mild to moderate in intensity. Unlike cluster headache, autonomic findings such as tearing and nasal discharge usually do not accompany it.

Before diagnosis, secondary causes of headache (high blood pressure, sleep apnea, intracranial lesions) should be excluded. Lithium, caffeine, and some anticonvulsants may be effective in treatment.

Symptoms

Headache waking the person at the same hours of the night
Pain being bilateral and dull in nature
Mild to moderate pain intensity
Attacks lasting from 15 minutes to several hours
More than 10 attacks per month
Usual absence of autonomic findings
Daytime fatigue resulting in drowsiness

Risk Factors

Being over 50 years old
Female sex
Sleep disorders
High blood pressure
History of previous headache
Changes in caffeine habits
Family history

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • If headache regularly wakes the person from sleep
  • If accompanying nausea or neurological symptoms are present
  • If new-onset nighttime headache appears after age 50
  • If progressively worsening headache is present
  • If quality of life and sleep are seriously affected
