What is Home Exercise Program in Rehabilitation?

Home exercise programs (HEP) are individualized therapeutic exercise prescriptions designed by physiotherapists and physiatrists for patients to perform between or after clinical rehabilitation sessions. They consolidate motor learning, prevent deconditioning, maintain therapeutic gains, support self-efficacy and contain healthcare costs.

Effective HEPs include a small number of targeted exercises (typically 3-7), clear written and visual instructions, dosing parameters (sets, repetitions, holds, frequency), progression criteria, safety precautions and a feedback mechanism. Modern programs use mobile apps, video and telerehabilitation platforms with real-time tracking. Evidence supports HEPs in osteoarthritis, low back pain, stroke, COPD, cardiac rehabilitation, falls prevention and post-surgical recovery.

Adherence is the main determinant of success. Strategies include shared goal setting, habit formation cues, gradual progression, exercise diaries, family involvement, periodic clinician review and digital reminders. Barriers include pain, fatigue, depression, lack of motivation, poor instructions and complex programs. Tailoring complexity to cognitive, physical and motivational level optimizes outcomes.
