What is HIFU (Focused Ultrasound) for Uterine Fibroids?

High-intensity focused ultrasound (HIFU) delivers focused ultrasound energy to uterine fibroids, producing controlled thermal ablation while sparing adjacent tissues. MR or ultrasound guidance provides real-time targeting and thermometry.

It is indicated for women with symptomatic fibroids who wish to avoid hysterectomy or myomectomy and meet anatomic eligibility (favorable fibroid type, size and location).

Symptom relief is generally achieved within 3–6 months. Reintervention may be needed in some patients, and pregnancy outcomes are still being studied.

Symptoms

Heavy menstrual bleeding from fibroids
Bulk symptoms (pelvic pressure, urinary frequency)
Pelvic pain associated with myoma
Desire to preserve uterus or avoid surgery
Failed medical management
Patient unfit for general anesthesia/surgery

Risk Factors

Subserosal/submucosal fibroids unsuitable
Pedunculated fibroids excluded
Body habitus limiting acoustic window
Extensive abdominal scars in beam path
Unstable to lie prone for hours
Pregnancy or active pelvic infection

  • Symptomatic fibroids with desire for uterine preservation
  • Failure or intolerance to medical therapy
  • Need for non-surgical alternative to myomectomy
  • Reduced fertility intent but not seeking immediate pregnancy
  • Recurrent fibroid symptoms post embolization

Treatment Methods

01
Patient screening with pelvic MRI
02
Bowel preparation and prone positioning
03
MR thermometry-guided sonication delivery
04
Postprocedural analgesia and follow-up imaging
05
Lifestyle advice for symptom management
06
Multidisciplinary discussion with gynecology and IR
