What is Hemifacial Spasm?

Hemifacial spasm develops when the facial nerve is compressed at its exit from the brainstem, usually by a blood vessel. Over time, involuntary, intermittent contractions appear on one side of the face. Initially limited to the area around the eye, spasms may spread to the face, lip, and neck.

The disease is generally seen in middle age and older, and is slightly more common in women. It worsens with stress, fatigue, and during speech; it may even continue during sleep.

Diagnosis is made with clinical and imaging findings; brain MRI is necessary to investigate neurovascular compression and other causes. Botulinum toxin injections are the most effective treatment option.

Symptoms

Involuntary twitches starting around the eye
Spasms spreading to one side of the face
Spasms increasing with speech and excitement
Involuntary closure of the eyelid
Involuntary pulling of the corner of the mouth
Cosmetic and social anxiety
Movements continuing even during sleep

Risk Factors

Vascular-nerve contact anomaly
Middle to advanced age
Female gender
History of Bell's palsy
Multiple sclerosis
Pontine lesions
History of trauma or surgery
