What is Telerehabilitation?

Telerehabilitation (Tele-Rehab) is the remote delivery of rehabilitation services using videoconferencing, asynchronous platforms, mobile health (mHealth) applications, and wearable devices for monitoring biomechanical and physiological data. Services include physiotherapy, occupational therapy, speech-language pathology, neuropsychological rehabilitation and home-based exercise supervision.

Models include synchronous (real-time video sessions), asynchronous (recorded exercise programs with feedback), and hybrid (combined remote and in-person care). Wearable inertial measurement units (IMUs), depth cameras and EMG biofeedback devices provide objective measures of range of motion, gait, balance and adherence. Artificial intelligence-driven exercise correction is increasingly available.

Strong evidence supports telerehabilitation in stroke, total joint arthroplasty, COPD pulmonary rehab, cardiac rehabilitation, chronic low back pain, multiple sclerosis and pediatric rehabilitation. Outcomes are non-inferior to in-person care for selected programs while improving access, adherence and cost-effectiveness. Limitations include digital literacy gaps, internet access disparities, hands-on technique limitations and reimbursement complexity.
