What is Scoliosis-Specific Exercises (PSSE)?

Physiotherapeutic Scoliosis-Specific Exercises (PSSE) are three-dimensional, asymmetric, scoliosis-specific exercise programs designed by trained therapists to address the unique curve pattern of each patient. Major schools include Schroth (Germany), Scientific Exercise Approach to Scoliosis (SEAS, Italy), Barcelona Scoliosis Physical Therapy School (BSPTS, Spain), Lyon (France), DoboMed (Poland), FITS (Poland) and Side-Shift (UK).

Common principles include 3D self-correction, axial elongation, asymmetric breathing (rotational angular breathing), postural alignment training, integration of corrected posture into activities of daily living and stabilization through neuromuscular control. Programs target prevention of curve progression, improvement of clinical presentation, respiratory function and quality of life.

Indications include adolescent idiopathic scoliosis (AIS) at low and moderate risk of progression, particularly during growth spurts, and adult scoliosis for pain management and progression prevention. PSSE is integrated with bracing in moderate curves (Cobb 25-40°) and may complement surgical decision-making. SOSORT (Society on Scoliosis Orthopaedic and Rehabilitation Treatment) guidelines recommend PSSE for AIS based on growing evidence base.

Risk Factors

Family history of idiopathic scoliosis
Adolescent growth spurt
Female sex (higher progression risk)
Skeletal immaturity (Risser 0-3)
Initial Cobb angle >20°
Connective tissue disorders
Neuromuscular conditions

When to See a Doctor?

  • Curve progression >5° in 6 months
  • New-onset back pain or neurological symptoms
  • Decline in respiratory function
  • Skin breakdown from bracing
  • Curves exceeding surgical threshold (>50°)
