What is Rheumatoid Arthritis Exercise Programme (PMR)?

Therapeutic exercise is a cornerstone of rheumatoid arthritis (RA) management complementing pharmacological therapy by improving aerobic capacity, muscle strength, joint mobility, balance and fatigue while preserving cartilage and reducing cardiovascular risk.

EULAR and ACR guidelines recommend a combination of aerobic exercise (walking, cycling, swimming, 30-60 min, 3-5 days/week), resistance training (8-10 RM, 2-3 days/week) and joint-range-of-motion exercises supervised by a physiotherapist; aquatic therapy is preferred during flares.

Programmes are individualised to disease activity (DAS28), joint involvement, comorbidities and functional capacity; tai chi, yoga and gentle Pilates are useful adjuncts for balance and well-being; principles include joint protection, energy conservation and graded progression.

Established RA on DMARD therapy
