What is Fall Prevention Program?

Fall prevention programs are evidence-based multifactorial interventions targeting modifiable risk factors in older adults at moderate-to-high fall risk. Key components include progressive strength and balance training (Otago Exercise Program, Tai Chi, FaME), home hazard assessment and modification, medication review (especially psychotropics, antihypertensives), visual assessment, footwear evaluation and education on fall prevention strategies.

Risk stratification uses validated tools including history of falls, Timed Up and Go test, Berg Balance Scale, 4-Stage Balance Test, gait speed and FRAX score. The STEADI (Stopping Elderly Accidents, Deaths and Injuries) framework by CDC offers a structured screen-assess-intervene approach. Patients with multiple comorbidities, polypharmacy and cognitive impairment require multidisciplinary management.

Effective programs report 25-40% reduction in fall rate and fall-related fractures. Otago program (5 sessions over 6 months with home-based progression) and group Tai Chi (60-minute sessions twice weekly for 6 months) are extensively validated. In Turkey, fall prevention programs are integrated into geriatric outpatient clinics and physiotherapy/rehabilitation services.
