What is Fall Prevention in Older Adults?

Falls occur in about 30% of adults aged 65 or over each year and are a major cause of morbidity and mortality. Hip fracture, head injury and activity-limiting fear of falling are serious consequences.

Causes of falls are intrinsic (age, muscle weakness, balance disorder, vision-hearing loss, orthostatic hypotension, polypharmacy) and extrinsic (environmental hazards, footwear, walking aids).

The CDC STEADI (Stopping Elderly Accidents, Deaths and Injuries) algorithm is used for fall-risk screening. Timed Up and Go, 30-second chair stand and tandem stance are commonly used assessment tools.

Symptoms

A fall in the past year
Gait and balance disorders
Fear of falling (fall-related self-efficacy)
Muscle weakness
Dizzy spells
Visual and hearing impairment

Risk Factors

Age over 65 and female sex
Polypharmacy (four or more medications)
Psychoactive drugs
Sarcopenia and muscle weakness
Orthostatic hypotension
Dementia and cognitive impairment

  • After any fall in the previous year

  • When gait speed slows
  • When fear of falling limits activity
  • For a polypharmacy review
