What is Dual Energy CT for Gout Crystal Imaging?

Dual energy CT (DECT) is an advanced CT technique that simultaneously scans at two different energy levels (typically 80 kVp and 140 kVp) to differentiate the chemical composition of materials.

Since uric acid crystals show different X-ray absorption from calcium, DECT specifically displays gout tophi and uric acid deposits on color-coded maps.

Used in differential diagnosis of arthritis, detection of subclinical gout, and treatment response monitoring.

Symptoms

Recurrent monoarticular arthritis (especially 1st metatarsophalangeal joint)
Chronic tophaceous gout disease evaluation
Uric acid deposit research in atypical arthritis presentations
Treatment response monitoring (tophus size reduction)
Differentiation of calcium pyrophosphate deposition disease
Crystal deposit screening in asymptomatic hyperuricemia

Risk Factors

Radiation exposure (similar to standard CT, 2-5 mSv)
Iodinated contrast not required (low risk)
Sensitivity limitation in small crystal deposits (<2 mm)
Subchondral erosion artifact (false positive)
Motion artifact in skin tophi
Composition limitation in kidney stones

When to See a Doctor?

  • Atypical arthritis with normal serum uric acid level
