What is Diffusion Tensor Imaging (DTI) MRI?

Diffusion tensor imaging (DTI) is an MRI technique that quantifies the magnitude and directionality of water diffusion in tissues, particularly along white-matter tracts where diffusion is anisotropic.

Common output parameters include fractional anisotropy (FA), mean diffusivity, axial diffusivity and radial diffusivity. Tractography reconstructs major fiber bundles for clinical and research use.

Clinical applications include preoperative tractography for tumors, evaluation of traumatic brain injury, multiple sclerosis, neurodegenerative diseases and developmental disorders.

Symptoms

Preoperative planning for brain tumors near eloquent areas
Suspected diffuse axonal injury after head trauma
Multiple sclerosis lesion characterization
Suspected leukodystrophy
Spinal cord cervical myelopathy
Neurodevelopmental disorder workup

Risk Factors

Cooperative patient (motion-sensitive)
Children may need sedation
Surgical implants must be MRI-compatible
Severe claustrophobia (relative)
Limited utility in markedly distorted anatomy

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Brain tumor near motor or language pathways
  • Persistent neurologic deficits after concussion
  • Atypical demyelinating disease
  • Pediatric developmental delay with imaging needs
  • Cervical myelopathy planning surgery

Treatment Methods

01
Acquire 30+ diffusion directions when possible
02
Standard MRI safety screening
03
Use deterministic or probabilistic tractography algorithms
04
Multiplanar review with FA color maps
05
Integrate with neuronavigation for surgery
06
Multidisciplinary correlation with neurology/neurosurgery
