What is Dry Needling for Trigger Points (Advanced Application)?

Dry needling (DN) is a manual therapy technique in which fine filament needles, similar to those used in acupuncture but without injectate, are inserted into myofascial trigger points (MTrPs) and dysfunctional connective tissue. Advanced application combines superficial and deep DN techniques with specific muscle palpation, ultrasound guidance and electrical stimulation (e-DN).

Mechanisms include local twitch response that disrupts dysfunctional motor end plates, neuromodulation of nociceptive pathways at spinal and supraspinal levels, and improvements in local blood flow and tissue extensibility. Indications include cervical and lumbar myofascial pain, tension-type headache, lateral epicondylitis, plantar fasciitis, and post-stroke spasticity.

Adequate training, anatomical knowledge and ultrasound guidance for high-risk regions (thoracic spine, intercostal areas, anterior neck) are essential. Common adverse events are post-needling soreness, bruising and transient symptom flare; rare complications include pneumothorax, vascular injury and nerve injury.
