DEXA Bone Density Measurement

What is DEXA Bone Density Measurement?

DEXA (dual-energy X-ray absorptiometry) measures bone mineral density (BMD) in g/cm2 using X-rays at two different energy levels. The radiation dose is very low and it is the standard examination.

Measurements are usually obtained from the lumbar spine L1–L4 and hip (total femur and femoral neck). Results are reported as T-score (comparison with the young-adult mean) and Z-score (comparison with same-age peers).

A T-score ≤ −2.5 is defined as osteoporosis and between −1.0 and −2.5 as osteopenia. Combined with the FRAX tool, the 10-year fracture risk can be calculated.

Symptoms

Screening in postmenopausal women
Women over 65 and men over 70
History of fragility fracture
Long-term corticosteroid use
Early menopause and hypogonadism
Malabsorption and malnutrition

Risk Factors

Pregnancy (relative)
Recent barium or nuclear medicine examination
Vertebral instrumentation
Severe obesity (device limits)
Improper positioning
Artifact in patients who cannot stay still

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Medical evaluation is required in any individual with fracture risk
  • Repeat scan every 2–5 years is recommended in those with negative initial DEXA
  • Periodic measurements are performed to evaluate response to treatment
  • Additional imaging is requested in case of new pain and suspected vertebral fracture
