What is Dentin Hypersensitivity (Detailed)?

Dentin hypersensitivity occurs when dentin is exposed due to enamel loss (erosion, abrasion, attrition, or abfraction) or gum recession, opening dentinal tubules to external stimuli. According to the hydrodynamic theory, fluid movement within these tubules excites pulpal nerve fibers, producing sharp pain.

It affects 10 to 30% of adults and is most common in the 20 to 50 age group, particularly in patients with periodontal disease, aggressive tooth-brushing, dietary acid exposure (citrus fruits, soft drinks, wine), bulimia, and bruxism.

Diagnosis requires excluding other causes of dental pain such as caries, cracked tooth, leaking restorations, and pulpitis. Cold air or water testing reproduces the pain typically lasting only seconds.

Symptoms

Sharp brief pain triggered by cold drinks
Pain with hot foods, sweet or acidic items
Pain on tooth-brushing
Pain on inhaling cold air
Pain limited to one or several teeth
Cervical (gumline) wear or recession
Pain stops when stimulus removed

Risk Factors

Aggressive horizontal tooth-brushing
Hard-bristle toothbrush use
Dietary acid exposure (citrus, soft drinks, wine)
Gastroesophageal reflux disease
Bulimia or eating disorders
Bruxism and tooth grinding
Periodontal disease and gum recession
