What is Dental Implants?

Dental implants are surgically placed root-form fixtures (typically commercially pure titanium grade IV or Ti-6Al-4V alloy, or yttria-stabilized tetragonal zirconia) that achieve osseointegration with the jawbone over 3-6 months. The implant body, abutment and prosthetic crown form the three-component restoration system. Modern surface treatments (acid-etched, sandblasted, anodized) accelerate bone integration.

Treatment phases include comprehensive clinical and radiographic evaluation (panoramic, CBCT), virtual surgical planning, surgical placement (one-stage or two-stage), osseointegration period (3-6 months), abutment connection and final prosthesis. Immediate loading is feasible with primary stability >35 Ncm and adequate bone quality. Computer-guided surgery and digital workflows improve accuracy.

Indications include single tooth replacement, multiple teeth replacement (bridges, full arch), complete arch rehabilitation (All-on-4, All-on-6, full-arch zirconia) and removable prosthesis support (overdenture). Contraindications include uncontrolled diabetes, smoking >10 cigarettes/day, bisphosphonate therapy (risk of MRONJ), inadequate bone volume (requiring grafting or sinus lift) and parafunctional habits. Long-term complications include peri-implantitis (10-22% prevalence), screw loosening, fracture and esthetic problems.

Symptoms

Complete edentulous arch with bone availability
Failed conventional removable prosthesis
Esthetic concerns with bridge or denture
Trauma-related tooth loss
Congenital absence of teeth (agenesis)

Risk Factors

Patient factors affecting success:
Uncontrolled diabetes mellitus (HbA1c >7-8%)
Smoking >10 cigarettes/day
Bisphosphonate therapy (bone necrosis risk)
Inadequate bone volume or quality
Active periodontal disease in remaining teeth
Parafunctional habits (bruxism)
Radiation therapy to head/neck
Immunosuppression and severe systemic disease
