What is Degenerative Cervical Myelopathy?

Degenerative cervical myelopathy is a progressive myelopathy due to narrowing of the spinal canal as a result of cervical disc degeneration, osteophyte formation, and ligament thickening. It is among the most common causes of neurological dysfunction over age 55.

The disease progresses slowly; deterioration of fine hand motor skills, gait difficulty, and loss of balance are typical. In advanced cases, spasticity in the limbs, urinary incontinence, and falls can be seen.

Diagnosis is made by clinical examination, cervical MRI, and electrophysiology. Surgical decompression is the cornerstone of treatment in progressive cases.

Symptoms

Loss of fine hand motor skills
Imbalance and slowing of gait
Weakness and numbness in the arms
Spasticity and hyperreflexia
Neck pain
Urinary urgency or incontinence
Risk of falling

Risk Factors

Advanced age
Chronic cervical degeneration
Congenital narrow spinal canal
Heavy physical work and repetitive trauma
Rheumatic diseases
Past cervical trauma
Ossified posterior longitudinal ligament
