What is Deep Brain Stimulation and Neuromodulation?

Deep brain stimulation (DBS) is a neuromodulation technique that delivers chronic electrical stimulation through electrodes placed in specific brain nuclei to modulate pathological neural networks.

It markedly reduces symptoms in selected patients with Parkinson's disease, essential tremor, dystonia, and treatment-refractory obsessive-compulsive disorder.

DBS is reversible and adjustable; the electrodes are permanent, but stimulation parameters can be programmed externally.

Symptoms

Persistent tremor and bradykinesia despite medical therapy
Severe motor fluctuations and on-off periods
Levodopa-induced dyskinesia
Drug side effects impairing quality of life
Refractory dystonia and muscle contractions
Compulsions limiting daily life in refractory OCD
Drug-resistant epilepsy and chronic pain syndromes

Risk Factors

Advanced age and concurrent dementia
Uncontrolled cardiovascular disease
Coagulopathy and bleeding disorders
Active infection
Severe psychiatric illness
MRI safety concerns around the implant

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Worsening Parkinsonian motor symptoms with inadequate medication response
  • Tremor not controlled by medication
  • Dystonia limiting daily life
  • Refractory obsessions and compulsions
  • Pain, redness, or malfunction in the implanted system
  • Mood or behavioral changes after stimulation

Treatment Methods

01
Detailed clinical and neuropsychological evaluation in a movement-disorders clinic
02
Surgical planning with MRI, DTI, and microelectrode recording
03
Electrode placement in subthalamic nucleus, globus pallidus, or VIM nucleus
04
Pulse-generator implantation and postoperative programming
05
Periodic stimulation adjustment and revision of medication doses
06
Wound and infection follow-up, and battery-life monitoring
07
Multidisciplinary rehabilitation and long-term follow-up
