What is Corticobasal Syndrome (CBS)?

Corticobasal syndrome is a progressive neurodegenerative syndrome with combined involvement of the brain cortex and basal ganglia, presenting with levodopa-unresponsive asymmetric parkinsonism and cortical function losses.

It usually starts over age 60; unilateral hand clumsiness, stiffness, and loss of fine motor control are the earliest complaints.

While the underlying pathology is often corticobasal degeneration (tau pathology), progressive supranuclear palsy, Alzheimer pathology, and TDP-43 proteinopathies can also lead to a similar clinical picture.

Symptoms

Unilateral hand clumsiness and stiffness in movements
Asymmetric rigidity and bradykinesia
Apraxia: inability to perform purposeful movements
Alien hand syndrome and uncontrolled limb movements
Focal dystonia and myoclonus
Cortical sensory loss (two-point discrimination, graphesthesia)
Speech disorders and dementia in advanced stages

Risk Factors

Advanced age
Family history of tauopathy group
MAPT gene mutations (rare)
History of neurodegenerative disease

  • Progressive loss of use of one hand
  • Sudden loss of control in hand movements
  • Loss of skills such as buttoning, writing
  • Asymmetric tremor or dystonia
  • Significant impairment in speech
