What is Coronary CT Angiography (Detailed)?

Coronary CT angiography is a high-resolution, ECG-gated multidetector CT examination focused on coronary artery anatomy and disease. It provides whole-heart imaging in a single breath-hold and offers exquisite negative predictive value above 95% for ruling out obstructive coronary artery disease.

Beyond stenosis evaluation, advanced post-processing enables coronary plaque characterization (calcified, non-calcified, vulnerable), Agatston calcium scoring, FFR-CT for ischemia assessment and CT-derived coronary angiography for procedural planning.

Symptoms

Stable chest pain with low-to-intermediate pretest probability
New-onset heart failure of unclear etiology
Atypical chest pain with abnormal stress testing
Pre-procedural coronary evaluation before non-coronary cardiac surgery
Coronary artery anomaly or fistula evaluation
Bypass graft patency assessment

Risk Factors

Diabetes mellitus and metabolic syndrome
Hypertension and hyperlipidemia
Family history of premature coronary artery disease
Smoking and obesity
Chronic kidney disease (relative caution)
Atrial fibrillation or uncontrolled arrhythmia (image quality)
Severe coronary calcification (blooming artifact)

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Stable chest pain not yet evaluated
  • Equivocal results on stress testing or echocardiography
  • Suspected coronary anomaly in symptomatic young patients
  • Recurrent symptoms after PCI or CABG
  • Risk stratification in asymptomatic high-risk patients (selected)
  • Pre-operative evaluation before structural heart procedures

Treatment Methods

01
Heart rate optimization with oral or IV beta-blocker
02
Sublingual nitroglycerin 1-3 minutes before scan
03
Prospective triggering at 70-80% R-R for radiation reduction
04
Iterative reconstruction and dual-source acquisition
05
Multiplanar curved reformations and 3D volume rendering
06
FFR-CT for hemodynamically significant lesion identification
07
Functional testing or invasive angiography for positive findings
08
Statin and antiplatelet therapy initiation when plaque is identified
