What is Coronary Calcium Scoring?

Coronary calcium scoring (CAC) is a quantitative method that measures the density and area of calcified plaques in the coronary arteries using a non-contrast, low-dose cardiac CT scan. The result is reported as the Agatston score.

It is used in asymptomatic intermediate-risk individuals to reclassify cardiovascular event risk and to inform treatment decisions (e.g., starting a statin). A score of 0 indicates very low risk and ≥400 indicates high risk.

The acquisition time is short, the radiation dose is low, and IV contrast is not required. However, it does not provide information on plaque stability or non-calcified plaques.

Symptoms

Asymptomatic intermediate-risk individuals
Those with a family history
Decision-making about lipid therapy
Risk assessment in diabetic individuals
Chronic kidney disease
Subclinical atherosclerosis screening

Risk Factors

Pregnancy
Active cardiac symptoms (not contraindicated, but CT angiography is more appropriate)
Very young individuals (no guidelines)
Those already classified as high-risk (unnecessary)
Motion artifact
Arrhythmia
