What is Constraint-Induced Movement Therapy (CIMT)?

Constraint-Induced Movement Therapy (CIMT), developed by Edward Taub, is a neuroplasticity-based approach used in rehabilitation of the affected upper extremity after stroke and traumatic brain injury. It targets the concept of 'learned non-use'.

In the classical CIMT protocol the unaffected hand is restrained with a mitt/glove for 90% of the day for 2 weeks, while intensive task-specific training is performed with the affected hand (6 hours per day for 10-14 days). Modified CIMT (mCIMT) involves a 3-hour daily and 30-hour weekly version.

Conditions: minimum motor criteria — 10° of active wrist extension and 10° of finger extension in the affected hand. In patients with lower function, modified protocols are applied.

Symptoms

Hand-arm weakness after stroke
Non-use of affected extremity (learned nonuse)
Asymmetric use in daily life
Chronic functional loss after stroke
Decreased hand dexterity
Habit of one-handed use

Risk Factors

Stroke
Traumatic brain injury
Cerebral palsy (children)
Multiple sclerosis
Spastic hemiparesis
Learned non-use

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • In subacute or chronic stroke
  • When minimum motor criteria are met
  • When upper extremity functional loss persists
  • When intensive rehabilitation is acceptable
  • When traditional therapy has plateaued

Treatment Methods

01
Motor criteria assessment
02
Restraining the unaffected hand (mitt/glove)
03
Intensive task-specific training 2-6 hours per day
04
Shaping and task practice
05
Home transfer package
06
Integration into activities of daily living
07
Modified CIMT (3 weeks)
08
Family education and social support
