What is Clinical Electromyography?

Electromyography (EMG) records electrical activity of muscle at rest and during voluntary contraction, while nerve conduction studies (NCS) measure conduction velocity and amplitude of sensory and motor nerves. Together they localize lesions and quantify severity.

Indications include radiculopathy (cervical and lumbar disc disease), peripheral neuropathy (diabetic, idiopathic, hereditary), entrapment syndromes (carpal tunnel, ulnar at elbow), motor neuron disease (ALS), myasthenia gravis, and myopathies (inflammatory, dystrophic).

Standard protocol assesses both sensory and motor NCS bilaterally with F-wave and H-reflex when needed, repetitive stimulation for myasthenia, single-fiber EMG for sensitive neuromuscular junction assessment, and needle EMG of representative muscles to evaluate spontaneous activity, motor unit morphology, and recruitment.
